LMR Update: Discharge Rx Printing Increases Efficiency/Patient Safety
SAFE AND EFFICIENT: To either write an electronic prescription or gain access to check the discharge medications, clinicians can simply visit the patient’s LMR, select the medication section and choose the new tab entitled, “discharge.”
Thanks to a web-based Longitudinal Medical Record (LMR) application developed at Partners HealthCare, clinicians now have the capability of printing patient prescriptions at the time of discharge from BWH.
This new function comes at a critical time when BWH is engaged in a statewide patient safety initiative geared to build awareness of medication safety, including transitions of care.
“For busy physicians, this new tool offers a faster, clearer and less error prone practice for discharge prescriptions,” said Robert G. Goldszer, MD, MBA, associate chief medical officer, who explains that this tool helps the hospital improve quality and safety
at a point in the hospital stay when a lot of information is being communicated to the patient. Additionally, this new tool allows for Partners ambulatory clinicians to easily check what medications the patient was sent home with by checking the patient’s electronic medical record.
To either write an electronic prescription or gain access to check the discharge medications, clinicians can simply visit the patient’s LMR, select the medication section and choose the new tab entitled, “discharge.” All of a patient’s discharge medications will be posted on that screen once the electronic discharge summary has been signed.
